Waivers and Forms
Core Team Retreat | September 7-8, 2019
Please submit these waivers and forms by Friday, August 23. Forms can be emailed to [email protected] or mailed to our office (see above address). Please have all forms postmarked by August 23.

- Health Insurance Card copies (front and back)
- Please submit copies of your health insurance card if (a) you are a first-time Core Team leader or (b) if you are a returning Core Team leader and your health insurance information has changed since you last submitted your card to us.
